This may happen - it is already in TODO to generate some more user friendly warnings.

Therion by default generate map in 1:200 scale (for caves), which is in your case (map at 20x30km a PDF with dimensions 100x150 m. Try using

-layout-scale 1 20000

or some even smaller scale.

Or use SVG output instead of PDF -- there is no arbitrary limit on page size. Martin
