[Therion] Entrance for cave list

As I said in my first email, I have used that, however, the thbook also
states that
entrance 0
Should work in survey, to define a cave in the cave-list assumably for
multi entrance systems then
station 0 "" entrance
can be used for other entrances.
The original email I sent gave your solution, but the thbook command as
stated in my original email does not work.
