[Therion] export cave-list

Thanks Bruce, this is working with your syntax.

I did not captured that specifying "-entrance RD21" as an option to the 
survey was actually different from specifying "station RD21 entrance" 
within the survey, which is what I did in all my surveys (and what was 
working to get the entrance list using survex tools if I remember well).

If this difference between the two is intentional, thenit would be very 
useful to have an explanation added somewhere in the Therion book.
