[Therion] Entrance for cave list

Page 17 of the Therion hook states that station <station name>
Under the context survey should be used as the entrance identifier for a
cave to be used in cave-list export.

However, this gives an error unknown command -- entrance

Now I can sort of get round it using
Station 0 "" entrance
in the centerline, but this does not define a cave so the list is a bit
weird. Am I missing something?
